Long Term Living Waivers


Our services support individuals who qualify for Pennsylvania’s Community HealthChoices (CHC) program. This initiative helps older adults and individuals with physical disabilities receive long-term services and supports in their homes rather than institutional settings. If you or your loved one is aged 60 or older, or living with a physical disability, you may qualify for in-home care through CHC.
